Commitment Agreement

It is agreed that Naomi A. Yehoshua, RDN will contribute top notch education, motivation, and the "blueprints" for results for the undersigned. In return, the undersigned will contribute honest effort, time, and desire to achieve the results agreed upon. 

It is agreed that only full and complete teamwork will achieve these results. It is also agreed that the undersigned will commit to contribute maximum effort without excuse or procrastination. Furthermore, it is agreed that Naomi A. Yehoshua, RDN will commit to seeing the undersigned to success. 

If initial payment shall be made at any day other than the first of the month, the monthly rate shall be prorated for the first months, and billed on the first of the subsequent billable month. 3% fee applies to credit card payment. No fee for zelle, check, or cash. Exact monthly dues determined on a case to case basis and shall be outlined in a separate document. 

This constitutes the entire Commitment Agreement between the undersigned and Naomi A. Yehoshua, RDN. This agreement will remain in force for a period of a minimum of six (6) months starting from the date of first payment, at which point the undersigned and Naomi A Yehoshua, RDN will reassess goals and recommit to a new goal and agreement.

This is a legally binding agreement which shall be considered entered into with the submission of information in below data fields.
